You could ask the kids what they would prefer. Beaches, riding elephants, rafting, visiting parks, tiger shows - lots of stuff they might care to see!
An extended stay in the Kanchanaburi area where the "Bridge on the River Kwai" tours go to would give you a chance to see elephants, ride rafts, River Kwai Bridge, Hellfire Pass, museum, etc.
A flight to Phuket would give you some beach time. While there you could visit "James Bond Rock" and some of the islands.
You could wrap your trip around what the kids like. With one week in Thailand I would have at least the first night or two in Bangkok and the last night back in Bangkok for shopping. This would give you the rest of the days in between for tours or trips.
From Bangkok you could fly to Chiang Mai. Tour the night market, shop and check out the local travel agents for tours to the Golden Triangle area, hill tribes, Long Neck Villages, and see some elephants too!
In Bangkok you can sign up for tours at travel agencies. Almost all of the travel agencies will have a sample book of tours for you to choose from.
You will be around water a lot when you are on the boat for the 5 nights. Maybe something that is not always on water will be a treat. Lots of options in Thailand.

If you only have a week in thailand u are best to go to one island and stay there otherwise u will waste too much time travelling to and from places. Ive been around thailand and the islands and the best one i would recomend with teenagers is Koh pipi as there is a lot to do there in beach activities and also hiking around as well. It has an excellent night life but very busy. if u wont a place a little less touristy than koh chang is good. u can do treks and elephant rides and also 1 day boat cruse to another island and also koh chang is 1 third the price of koh pipi and it has an awsome night life but also very family friendly.
Go to Ko Samui, Ko Tao, Then Ko Pangang
Amazing landscape vegetation and people, kids will love it.
